By clicking this link you will have access to plan overviews and plan details. In the detail section, you will get expanded information and you can even download a plan's documents.
There is also a section for you to input your providers and prescriptions and you will see which plans have your providers in-network and which prescriptions will be covered.
After browsing, if you have specific questions, you can always call or email us.
If you would like to meet by phone or in person with either of our brokers you may request an appointment by calling us or submitting the form on our contact page.
Before a broker can meet with you discussing any Medicare type plan by phone or in-person, you must sign a scope of appointment form, giving them permission to talk about the type of plan you would like to discuss. Such as: Supplements, Advantage Plans, Prescription Drug plans, etc.
There is a downloadable scope of appointment form below that you can print and sign at home, or you can request that the broker sent you an electronic scope to be delivered by email or text. Both email and text scopes of Appointments can be digitally signed through your email, phone, or other device.
A Scope of Appointment form or (SOA) for short is a document required by Medicare to be signed by the Medicare Recipient 48 hrs prior to an appointment with a sales agent. and before that agent can discuss benefit information, like premiums, costs, copays and coinsurance.
The purpose of the Scope of Appointment form is for you to have the power to only talk about the types of plans you are interested in and not be presented marketing material on types you aren’t.
You must initial or check the products you would like to discuss and sign the form. Agents are not allowed to tell you what boxes to check.
Signing a SOA does NOT obligate you to enroll in a plan or change your plan. It only provides legal proof that you asked that agent to present that material and answer your questions.
